Following documents are required for:-  

1.    Birth registration:

(a)     Original birth certificate issued from Hospital
(b)     Proof of parentage
(c)     Proof of Indian citizenship of father &mother
(d)    Proof of residence/ workplace of parent.

2.    Death registration:
    
(a)     Medical certificate of a doctor 
(b)     Copy of police report
(c)     Proof of Indian citizenship
(d)     Copy of Work permit/ Dependent Card/ Trade License or proof of working in Bhutan
(e)     A letter from the organisation where the deceased was serving.
